This process must be completed by an employeee who is authorized to enter your company into legally binding agreements.

• Company Tax ID Information

• Insurance broker contact information or copy of Certificate of Insurance with the following minimum requirements:

• Auto Liability of $1,000,000 or greater

• Cargo Liability of $100,000 or greater

• Hazmat Certicate (if certified)

• Other Certifications, such as CARB, SmartWay, UIIA (if applicable)
